The name comes directly from the Greek δέσποινα (despoina), meaning “lady,” “mistress” or “lady of the house,” and is the feminine counterpart of δεσπότης (despotes), “master” or “lord,” from which the English word despot ultimately derives. The first element can be traced to the Indo-European root *dem-, meaning “house” or “household.”
In Greek mythology, Despoena was the title or epithet given to a mysterious Arcadian goddess, the daughter of Demeter and Poseidon. She was worshipped in a mystery cult in Arcadia, particularly at Lycosura. According to Pausanias, Despoena, meaning “the Mistress,” was not her true name; her actual name was revealed only to initiates and has consequently been lost.
The Greek title Despoina/Despina was also adopted into Ottoman and other Turco-Muslim court usage as a designation for certain high-ranking Christian women. The title occurs in the form Despina Hatun, combining Greek despoina, “lady, mistress,” with the Turco-Mongol title hatun, “lady.” It was notably applied to several Christian princesses married to Muslim rulers, including Olivera Lazarević, the Serbian wife of Ottoman Sultan Bayezid I, and Mara Branković, the Serbian wife of Sultan Murad II.
In modern Greek, Despoina (Δέσποινα), commonly rendered Despina, is also used as a title for the Virgin Mary, essentially meaning “Our Lady.” As a result of this association, Despoina/Despina remains in use as a feminine given name in Greece and its usage has spread to other neighboring Eastern Orthodox countries.
The name has a long history of usage in Romania, where Despina was originally used as a title for the wife or daughter of a despot. It initially accompanied a woman’s personal name, but as the original meaning of the title became less transparent, Despina eventually developed into an independent given name.
It was borne by Milica Despina (c. 1485–1554), a Serbian noblewoman who became Princess Consort of Wallachia through her marriage to Neagoe Basarab and served as Regent of Wallachia from 1521 to 1522 on behalf of her son, Teodosie.
The name appears in Wolfgang Amadeus Mozart’s 1790 opera Così fan tutte, in which Despina is the clever and mischievous maid of the sisters Fiordiligi and Dorabella. A principal character in the opera, she becomes an accomplice in Don Alfonso’s scheme to test the sisters’ fidelity, disguising herself first as a doctor and later as a notary. The role is written for a soprano and remains one of the best-known uses of the name Despina outside the Greek-speaking world, despite this, the name never came into widespread use outside the former Byzantine Empire.

Despina Achladiotou (1890–1982), better known as the Lady of Ro, was a Greek patriot and member of the Greek Resistance during World War II. She became famous for raising the Greek flag every day on the small island of Ro and was buried there with full military honors.
Despina Vandi (b. 1969) is a Greek singer and one of the most successful performers of modern Greek laïko and pop music.
Despina Olympiou (b. 1975) is a Greek-Cypriot singer who represented Cyprus at the Eurovision Song Contest in 2013.
Despina Papamichail (b. 1993) is a Greek professional tennis player who represented Greece at the 2024 Summer Olympics.

Despo (Δέσπω) is probably the most important traditional short form. It has also become an independent given name in Greece. A famous historical bearer was Despo Botsi (Δέσπω Μπότση), a Souliot woman celebrated for her resistance to Ottoman forces in 1803.
